Bank Membership Agreement
If you wish to cancel your membership plan with us due to a change of circumstances, we request that you provide one months notice in writing. Upon receiving this request, we will cancel all upcoming subscription payments on your behalf.
Cancellation of a membership plan during an active treatment course will remove all membership discounts from your treatment plan. The owed balance will require payment before your treatment plan is complete. Failing to make this payment could result in a delay to your treatment or cancellation of your treatment plan.
Failed membership payments
If your monthly subscription payment fails for any reason, we will endeavour to contact you as soon as possible to remake this payment. When a subscription payment fails, your membership benefits will be automatically halted until payment has been cleared.
If multiple membership payments fail, your Bank Membership plan will be cancelled. Please note this will affect all patients connected to your subscription. Any future appointments at the practice would be on a pay as you go basis.
Amending your Membership Plan
You can contact Bank Dental to discuss your membership agreement and advise us of any changes by the following methods:
Failed Appointments & Late cancellations:
Membership appointments:
If you fail to attend your membership exam/hygiene appointment or cancel with less than 24hours notice, this appointment will not be re-bookable under the benefits of your membership plan. If you wish to rebook your cancelled/missed appointment, our private fees will apply for this appointment. Your Bank Membership plan will then resume in line with your next recall interval.
Treatment appointments:
If you fail to attend or cancel an appointment with less than 48hours notice, you will be charged 50% of the planned appointment fee. This payment will need to be made to rebook the appointment. If you fail to attend or short notice cancel further appointments, we reserve the right to increase the charge applied.
Booking of Treatment appointments:
We kindly request that treatment appointments are paid for in advance, either in full or with a 50% deposit. We are unable to schedule appointments without receipt of pre payment. If you would like to discuss your payment options further, please contact us and ask to speak to our Treatment Coordinator.
Refusal of treatment:
Treatment may be refused if the treating clinician deems that treatment to be unsuitable or inappropriate.
