Bank Membership Agreement
If your circumstances change and you wish to cancel your membership plan with us, we request that you provide one full calendar months’ notice in writing. We will then cancel upcoming payments on your behalf.
Cancellation of a membership plan during an active treatment course will revert treatment costing to non-member prices. The owed balance will require payment before treatment is complete. Failing to make this payment could result in a delay to your treatment or cancellation of your treatment plan.
Failed membership payments
If multiple membership payments fail, your membership plan will be cancelled. Please note this will affect all patients connected to your subscription. All upcoming appointments will be cancelled, and you will only be able to return to the practice on a pay as you go basis.
Variation of conditions
From time to time, we may need to vary the conditions of this agreement to reflect changes in law or regulations. In these situations, you will receive correspondence or notification of any changes made. You can contact Bank Dental to discuss your membership agreement further, or advise us of any changes by the following methods:
Failed Appointments & Late cancellations:
Membership appointments:
If you fail to attend an appointment or cancel with less than 24hours notice, this appointment will not be re-bookable under the benefits of your membership plan. If you wish to rebook your cancelled/missed appointment, please note this will be a chargeable booking.
Treatment appointment:
If you fail to attend or cancel with less than 48hours notice, initially, you will be charged 50% of the planned appointment fee. This payment will need to be made to rebook the appointment. If multiple appointments are missed or short-noticed cancelled, 100% of the planned appointment fee will apply and payment will be required before rebooking the appointment.
Booking of Treatment appointment
To book any treatment appointments or to commence treatment plans, we kindly request a minimum 50% deposit of the planned appointment fee or in full, depending on the nature of planned treatment.
Refusal of treatment
Treatment may be refused if the treating clinician deems that treatment to be unsuitable or inappropriate.
